HDT Action Plan Evidence Log & SLAPPED - Public forum - Planning Advisory Service (PAS)

I'm a little confused as to how the Evidence Log and SLAPPED fit into the structure of the HDT Action Plan. Any examples people can share which can help me grasp how the Evidence Log and SLAPPED fit into the Action Plan would be good.

I'm muddling along but I don't really have a clue what I'm doing and that's after looking at the workshop slides and pilot HDT's.
