Monitoring Frameworks?

Hi,

I have been tasked at looking the 'monitoring' aspect of our emerging development management policies? I have been trying to find some best practise that have gone through         examinations.Does anyone have know of an agreed methedology or good practices?

 Despite good intentions, it seems to be done at the end of the process!
