Case study

The  Redbridge  Prevention  and Early Intervention  Strategy builds  on  existing commissioning  strategies  and  plans  to  improve  the  health  and  wellbeing  of  the  local community, by putting them at the centre of their care and providing support in an integrated way.  It highlights areas  where  shared  care  pathways and  outcome based commissioning  can improve  service  quality, productivity  and the service  user and carer experience. The Council entered into a Section 75 agreement with Redbridge CCG to access the Better Care Fund (BCF) monies and deliver the programme of work.
