CIL Reg 103 - Charging order - Public forum - Planning Advisory Service (PAS)

This Reg requires us to notify the debtor of our intention to make an application to the Court for a CO. 

I cannot find a statutory template for this and was hoping that maybe someone on here wouldn't mind sharing one, if they have produced one already?
