Scale of Engineering Ops under a GPDO

Does anyone know if there are any guidelines on the scale of engineering operation that can be reasonably carried out in forming an access as part of a GPDO activity before they are considered to require planning permission in their right? For example, would removing some 5 metres of a 2.5 metre elevated bank to access an unclassified road in a conservation area be generally considered acceptable in terms of Class B? The only insight I can find is in Appeal case APP/T3915/X/08/2084707 where the inspector regarded the removal of 10 - 15 cu metres of earth to create an opening as not going beyond those envisaged by Class B.
